After being put in to bat first, the Falcons posted 163 runs for the loss of four wickets in their innings.
Jewel Andrew remained unbeaten with 50 runs off 30 balls, while opener Fakhar Zaman added 43 runs off 32 balls, hitting five fours and two sixes.
In the chase, the Patriots struggled early, losing wickets regularly, and found themselves at 159 for nine in 19.3 overs. With just one run needed off the final ball, Nortje clinched the victory with a decisive single.
Mohammad Amir and Imad Wasim each took a wicket, conceding 31 runs in their four-over spells.
Shamar Springer claimed four wickets in his spell, while Roshon Primus secured two wickets.
